Description
For patients of Dr. Janalee Stokken, Dr. Erin O'Brien and Dr. Garret Choby to submit information about their sinus disease symptoms periodically. This is not a substitute for routine clinical care but responses may be used to track their symptoms of rhinosinusitis and plan future care. Responses will be used for research purposes (MCR IRB# 16-001409.)
If your symptoms are worsening, in addition to using this survey, please also contact your provider to schedule a follow-up appointment as soon as possible. If you have an urgent medical problem, please call your rhinologist, your primary care provider or an urgent care facility immediately.
